Günther Brammer 4b4b8781a0 Make GTK+ and SDL2 work together
If the gamepad code initialized the SDL video subsystem, GTK+ crashed in
libX11. Or something along those lines.

Making the optional subsystems using SDL for gamepads and audio use
SDL_InitSubSystem and only the SDL Application port do the full SDL_Init
is the proper way to do things in any case.

Sven Eberhardt 1cf188bb45 Fix music system running into infinite pause after playlist changes #1460 #1485
A playlist change would reset SCounter and eventually cause all songs to be non-eligable.

Since playlists can now select differently sized subsets, the old way of selecting random songs without too much repetition no longer works. Rewrite to use a more robust algorithm.

Nicolas Hake 24b54211c5 Drop Min, Max, Swap for std::min, std::max, std::swap
The C++ standard library comes with perfectly fine implementations of
these functions, so there's no point in reimplementing them just for the
hell of it.

Nicolas Hake 0c1abe69f9 Get rid of FMOD
Not only is FMOD neither free (libre) nor free (gratis), the version we
support(ed) is also impossible to legally obtain anymore. So there's no
reason we should keep code around that (pretends to) support a library
nobody can use or test.

Sven Eberhardt 3d7d848eea When fading from running songs, fade out and then start new songs directly instead of fading into them.
For songs that start abruptly, fading into them often ruins the beginning.

Sven Eberhardt baf05264d1 Implement music fading.
Moved music system execution from game tick execution into main application execution so music fading works when the game is paused or lagging.

Nicolas Hake 642ce2eb9c Merge all of the audio provider switches into one
There's no point in splitting the audio library selection into multiple
CPP macros, since there can always only be one anyway. Merge all of them
into a single macro AUDIO_TK (for "toolkit") and have CMake select one
for the user, instead of making him choose (and potentially failing).

Nicolas Hake e0dc30c59a Update copyright notices
As discussed in http://forum.openclonk.org/topic_show.pl?tid=2917, I
have merged all copyright notices into a single file and referenced that
merged file from each source file.

For the updated source files, the timeline has been split into three
parts:
 1. Pre-RWD code (before 2001)
 2. RWD code (2001 through 2009)
 3. OpenClonk code (2009 and later)
All pre-RWD copyright notices have been left intact, as have RWD-era
copyright notices where the file did not have a RedWolf design copyright
notice but only individual author ones. All copyright notices of the
OpenClonk era have been replaced by a single notice ranging from the
first recorded year to the current year (2013). Mape code did not get a
OpenClonk Team copyright notice because it is somewhat separate from the
main OpenClonk codebase and has only been touched by Armin Burgmeier.

Günther Brammer be342d4a51 copyright notices update 2010

Günther Brammer b4e37b070b Change all indentation with spaces to use tabs instead
This is a whitespace-only patch. Hopefully, it'll only affect rarely-changed
parts of the engine, since all regularly maintained pieces should already
use tabs.

Günther Brammer 4f08312df7 Script: Music() no longer overwrites the music settings of the user
If you want to stop music, you need to use SetPlayList() with a nonempty
playlist which matches no music files, " " is usually a good choice.

Nicolas Hake 0dc14ae40e win32: Unbreak fmod, remove MCI
USE_FMOD define renamed to HAVE_FMOD to bring it in line with HAVE_SDL_MIXER.
MCI removed since nobody was using it anyway, and half of its functions were
stubs.
