Huw Davies 4a8e33954e We need to send TVN_EXPAND* on collapsing either when the user has 
initiated it with the mouse/keybaord or in response to a TVM_EXPAND
when the state is not TVIS_EXPANDEDONCE.  This brings the logic into
line with TREEVIEW_Expand.

Adam Gundy a982259e3e When items are inserted in a partially populated treeview, the first 
visible item was not correctly updated.
The first visible item was sometimes used to calculate the location of
other items when it had been freed.
